Sioux City - Pharmacist - Half time

Due to business growth, independent, pharmacist-owned long term care pharmacy (no retail) seeking qualified staff pharmacist for half-time (average 20 hours per week) position. Summary:
Provides pharmaceutical services to long term care and institutionalized patients in a manner that maximizes quality and patient safety. Participates in and oversees the dispensing of medication in accordance with federal and state regulations and ValuMed Pharmacy policy.

Qualifications:
Education: Bachelor's or Doctorate Degree in Pharmacy.
Licenses/Certification: Current and active pharmacist license in the State of Iowa. Experience: One year experience in long-term care preferred.
Essential Functions:

  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
  • Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
  • Ensures that appropriate laboratory tests for monitoring certain high risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team.
  • Collaborates with the healthcare team to provide clinical and technical support as needed.
  • Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.
  • Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs.
  • Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and ValuMed Pharmacy policy.
  • Informs the Pharmacy Manager of medication related errors.
  • Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.
  • Actively participates in Performance Improvement activities. • Performs other tasks assigned.
  • Conducts job responsibilities in accordance with the standards set out in the Company's policies and procedures, applicable federal and state laws, and applicable professional standards.

Knowledge/Skills/Abilities:
  • Knowledge of pharmacy laws and federal and state regulations concerning long- term care.
  • Ability to work cooperatively as a member of a team.
  • Knowledge of pharmaceutical care practices for long term care residents.
  • Ability to maintain detailed and accurate records.




Sioux City - Pharmacy Data Entry Technician

An excellent opportunity to work in a long term care pharmacy. FT position available. Experienced owners will provide a dynamic non-retail environment with no nights or holidays. Retail or Hospital experience welcome. We offer a highly competitive wage structure. Must be a certified pharmacy technician in the state of Iowa or eligible to become certified.

Summary:
Assists with drug preparation, dispensing, and data entry under the direct supervision of a Pharmacist and in accordance with state pharmacy regulations.

Qualifications:
Education: High School Diploma or equivalent.
Licenses/Certification: State of Iowa certified technician or ELIGIBLE TO BECOME CERTIFIED.
Experience: Long-term care experience preferred but not required.
Essential Functions:

  • Assists the Pharmacist in filling prescription items, as permitted by the State Board of Pharmacy.
  • Types prescription information to produce labels.
  • Labels products appropriately, including cautionary or auxiliary labels.
  • Takes inventory, places orders, checks in drugs and supplies, stocks shelves and removes out-of-date items from the inventory.
  • Inputs patient data and medication orders into a computer system.
  • Prepares medications including emulsions, liquids, powders and ointments.
  • Packages medications for dispensing and for delivery.
  • Communicates with nursing center personnel and other customers in a pleasant and professional manner.
  • Processes returned medications from the facility for credit or destruction.
  • Performs other tasks assigned.

Knowledge/Skills/Abilities:
  • Ability to work as a member of a team.
  • Ability to operate personal computers and/or computer terminals.
  • Skilled in performing detailed work.
